Howard A. Zucker, MD: Just What the Patient Ordered

Neither the price of chocolates nor flowers will skyrocket this Friday as the nation celebrates Doctors' Day. Since its inception in 1933, commemorating the March 30, 1842, discovery of anesthesia by Crawford Long, the medical profession has undergone astounding transformations. In 1933, the Nobel Prize in Medicine went to Thomas Morgan for a revolutionary concept now taught in grade school -- that chromosomes play a role in heredity. We soon will be able to map our genomes for a thousand dollars as a result of DNA sequencing and personalized medicine.

But how personal is medicine today? Was the 1960 television's Marcus Welby-style general practitioner better able to treat the entire patient than TV's present day self-styled diagnostic-genius Dr. House?

As current physicians age and become patients themselves they are experiencing the health system from the other side of the bedside. They are part of the new old. Of the nearly one million doctors in America today, close to 200,000 are senior citizens. They benefit from Medicare, a program enacted during their prime years of practice. Now, with the legality of the Obama administration's Affordability Care Act before the Supreme Court, the implementation of electronic medical records, and the marvels of face transplantation no longer science fiction, the opinions of a vanishing breed of doctors on these and other changes may be worth hearing.

It is time for America to take directional cues from Steven Spielberg by creating an oral history of the medical profession. These gray-haired, furrowed-brow physicians may illuminate how we might meld the best of the past and the promise of the future of medicine.

In 1960 the majority of doctors were in solo practice, providing attentive quality care. However, solo practice grew victim to overwhelming insurance forms and other paperwork and unwieldy federal regulations. Today, less than one in five doctors are solo practitioners, according to the American College of Physicians. Instead, large groups, hospitalists, and physician extenders are the front line of medicine. Even the title, "doctor" is morphing into the generic "provider." Commercialization of health care is evident with billboards advertising everything from cardiac surgery to treatment for urinary disorders.

The elder physicians can provide insights about a profession that now markets itself in the same vein as the fashion and food industries. But their advice could focus on more than just clinical practice. The Institute of Medicine reported in 2009 that 18,000 deaths per year are attributable to lack of insurance. With a reported 50 million Americans uninsured according to the United States Census, the Affordable Care Act promises a 50% reduction. Physicians with decades of experience clearly have a perspective on government intervention -- both good and bad -- in health care delivery.

For example, the government implemented the Surgeon General's warning on cigarette boxes in 1971 at a time when physicians were championing President Nixon's fight against cancer. There must be useful knowledge regarding preventive health that doctors of that era could share as it relates to the present government efforts to combat childhood obesity.

Similarly, our medical education system would profit from some historical viewpoints. The American Association of Medical Colleges estimates that nearly 16,000 newly minted doctors will enter residency training this year. Unlike their mentors who generally acted more like Dr. Welby, they have been educated in a curriculum that tackles medical mysteries with a technological bend -- like Dr. House. Diagnoses once made through history and physical examination, then confirmed by laboratory tests, has been turned upside down. Our nation's teaching hospitals rely on $9 billion per year of Medicare funding which remains in jeopardy. How best to educate and fund new doctors joining the profession requires insight from those soon to retire.

The time has come to assemble doctors who practiced the art during an era with the greatest changes in the history of medicine -- from the discovery of antibiotics to routine open-heart surgery and organ transplantation today. Their reflections on the past can help us with solutions to better health care. The White House and Congress should announce a meeting of the old guard to hear their collective views on timely topics. We all will be beneficiaries if our leaders carefully consider their wisdom as America crafts a 21st century health care system. It may just be what the patient ordered on this Doctors' Day.

Howard Zucker, MD, JD is a pediatric cardiac anesthesiologist at Albert Einstein College of Medicine in the Bronx, New York and former Deputy Assistant Secretary of Health and former Assistant Director-General of the World Health Organization.

Megan Fox disputes 'unattractive girl' quote

By Us Weekly

Updated, noon PT: Megan Fox may be one of the hottest women in Hollywood -- but she wants to make it clear that she doesn't see herself that way.

Facebook

The 25-year-old "Friends With Kids" actress is featured on the cover of French magazine Jalouse's April issue. In their interview with her, Fox is quoted as saying she "live[s] well with my image. I cannot complain. I would not trade my place with an unattractive girl."

An annoyed Fox took to her Facebook page Friday to clarify her comments. "Sigh. The actual comment, the context and its intention are grossly distorted and misrepresented here," Fox writes on her profile page (above), adding that poor translation is really to blame. "I gave this interview in English obviously," she says. "It was then translated to French and now back into English."

The actress insists the quotes are completely out of character for her, and says she's not the type to make the type of "vapid self serving comments" printed by the magazine. In fact, Fox asserts, "[I am] in contradiction...uncontrollably self deprecating."

Earlier this month, Fox opened up to MIAMI magazine about feeling insecure and lonely as a teen. "I was never the pretty girl," she shared, calling her younger self "obnoxious" and "abrasive."

NAR: Low Real Estate Prices, Desire for Family Retreat Propel ...

AUSTIN, Texas, March 29, 2012 /PRNewswire/ ? In a sign of an emerging recovery in the real estate market, sales of vacation homes in 2011 reached a four-year high, powered by the lowest median sales price in eight years, low mortgage rates, and the desire for a family retreat, according to proprietary research commissioned by HomeAway, Inc. as part of the National Association of Realtor?s (NAR) 2012 Investment and Vacation Home Buyers Survey?.

The research found 33 percent of vacation home buyers purchased a vacation property last year primarily because of low real estate prices, while another 30 percent cited the desire for a family retreat. And 2011 proved an opportune time to buy, according to the NAR survey, with the median sales prices for vacation properties at $121,300, putting ownership within greater reach of more consumers.

Other vacation home buyers cited the following factors as the most important reason to take the plunge into vacation home ownership last year, according to HomeAway? research:

  • Low mortgage rates (13%)
  • For future retirement (11%)
  • Potential for price appreciation (8%)
  • Other (5%)

?The purchase of vacation homes is moving back into the mainstream for a greater number of Americans who are attracted by low prices, rental income potential, and use as a retreat for busy families to get together every year,? says Brian Sharples, chief executive officer of HomeAway. ?Although sales of vacation homes are not as high as historic levels, they are clearly showing signs of a sustained recovery.?

According to the NAR survey, 82 percent of buyers believe now is a good time to purchase real estate, and 39 percent say they are somewhat or very likely to buy vacation property within the next two years.

Vacation Home Owners Believe Rental Income Can Help Offset Mortgage on Second Home

Nearly all (91%) vacation home buyers say they plan to rent their property within the next 12 months to either long-term or short-term renters or a combination of the two. Of those buyers, 71 percent cited rental income potential as a factor in their purchase decision and nearly three-quarters (74.5%) of buyers believe they will make enough rental income to cover at least half of their mortgage.

According to Sharples, HomeAway research conducted in Q4 of 2011 of 4,905 owners listing their vacation homes on HomeAway?s U.S. sites show that 70 percent generate enough rental income to cover half or more of their mortgage, and 51 percent cover at least three-fourths of their mortgage.

Of those buyers intending to rent their property, about 40 percent plan to make their vacation homes available for rent between one and eight weeks over the course of the next year; 32 percent plan to rent their properties between nine and 26 weeks per year; and 27 percent plan to rent their homes between 27 and 52 weeks per year.

Vacation home buyers are willing to rent their property to more than one type of guest. The majority (70%) of people who plan to rent their property to short-term renters say they?ll rent the home to vacationers, while 37 percent plan to rent to business travelers and 24 percent plan to rent to other tenants, such as college students or people relocating to the area.

By the Numbers:

Highlights from the NAR 2012 Investment and Vacation Home Buyers Survey

?

502,000 ? Number of vacation homes sold in 2011 $121,300 ? The median sales price of a vacation home in 2011, down 19% from 2010

ZTE Fury Review

ZTE Fury

Could a device that is only $19.99 on a two-year contract hold any relevance in the Android world in 2012? It seems that lately you can either drop a couple of hundred dollars to get yourself a highly spec'd device or you can take the cheaper route and grab a mid range device, but what do you sacrifice by doing that? ZTE has been trying to become more reputable, they recently also released the ZTE Warp, and now they have brought the ZTE Fury to Sprint.

On paper the device has specs of what we saw in most 2011 devices, so that leaves us wondering -- is the ZTE Fury a year late to the party? Let's hit the break and check out how the device performed as it was put through the paces, and see if it could be enough to make it as your next daily driver.

The Good

For the price the ZTE Fury is a moderately spec'd Android device. The 3.5-inch screen is one that many will enjoy, it allows the device to have a nice small overall feel.

The Bad

Locking into a device like this on contract will ensure to keep you far behind on the Android technology. Camera is pretty bad, and barely even worth using.

Conclusion

If you are in the market for an entry-level device, and having the latest and greatest device isn't a big thing for you this could be a great option. The ZTE Fury is priced at a point to sell, but price shouldn't be everything.

Getting the Right Bandwidth For Your Video Conferencing ...

Video conferencing assuredly encompasses a range of technologies used in a wide range of situations, often it is not just video and audio that is transmitted, but also data, allowing collaborative working though shared applications. All of this means having adequate bandwidth supporting your network is the indispensable operation factor.

Video conferencing may be?

* One-to-one meetings, also known as point to point communications, normally spicy full two-way audio and video.

* One-to-many spicy full audio and video broadcast from the main site, where other sites may be able to send audio. For example in a lecture situation, students could ask questions.

* Many-to-many, known as multi-point communication, provides audio and video in the middle of more than two sites. With most multi-point systems only one site in a discussion can be seen at time, with switching in the middle of sites whether controlled manually or voice activated (i.e., the loudest site is on screen).

Physically, the most coarse scenarios of video conferencing are:

* desktop video conferencing ? normally a small camera is located on top of the Pc or workstation monitor. The actual video is normally displayed in a small window, and shared applications, such as a shared whiteboard are often used.

* studio-based systems ? a studio is specially adequate for video conferencing. This will normally contain one or more cameras, microphones, one or more large monitors, and perhaps other tool such as an overhead camera for document viewing. normally used for more formal meetings

In custom a ?studio? may not be a dedicated room, but a standard discussion room with portable tool that can be set up when required.

Bandwidth and Compression?

The bandwidth, or baud rate, is the whole of facts which can be transmitted every second. The higher the bandwidth, the better quality the signal that can be transmitted. For a video discussion audio and video signals must be transmitted in real time, i.e., a lot of facts has to be sent every second, requiring a very high bandwidth. For example a ?true colour? image will need 24 bits (3 bytes) per pixel. A full screen image might be 640?480 pixels, over 7 million bits. For full petition video, the image is refreshed 25 times per second. This adds to over 184 million bits per second. It is not realistically possible to transmit this whole of information, and your Pc assuredly could not receive it at this rate. Therefore for digital video some form of compression is required. The type and degree of compression used varies from system to system. It is spicy to note that for most uses, we are more tolerant of poor video than poor audio, and so some systems merge on providing consistently good audio.

How To Get The Right Bandwidth To Meet Your Needs?

Don?t take a chance at guessing what bandwidth you?ll need?or in choosing the victualer for that bandwidth. You may need a fractional, full, integrated, or bonded T1, Ds3, or Oc3 network depending on many factors. Solutions offered by providers will vary by cost, quality of assistance (QoS), and assistance Level agreement (Sla). I strongly propose you use the services of an independent Telecommunications counselor to help you assess your needs and find the victualer who best meets your requirements. For unbiased free advice?.as well as real time rate quotes from multiple providers, indepth research, and negotiations with providers on your behalf?.I propose you use FreedomFire Communications.

(AP) ? A Michigan militia leader and his son pleaded guilty Thursday to possessing a machine gun, giving prosecutors their only gain in a domestic terror trial that was upended when the judge dismissed charges of plotting war against the government.

Hutaree leader David Stone rocked in a chair at the defense table after pleading guilty and told reporters he was a "stand-up true American patriot" whose anti-government comments and bravado about wanting to kill police were not a call to attack the United States.

He and six militia members were cleared Tuesday of conspiracy charges, which he called a "victory for everyone" who cherishes the First Amendment.

"It's amazing how someone can take a comment out of context and make it to whatever they want it to be," Stone, 51, said.

Gun charges were all that remained for Stone and Joshua Stone, 23, both from Lenawee County, Mich., after U.S. District Judge Victoria Roberts said prosecutors in six weeks had failed to present evidence of a specific plan to go to war against law enforcement and federal authorities.

Jurors heard hours of secretly recorded conversations between David Stone and an FBI informant and agent. He talked about killing police, building bombs and engaging an international coalition of freedom-hating law enforcers dubbed the "brotherhood."

"The government's case is built largely of circumstantial evidence," the judge said Tuesday. "While this evidence could certainly lead a rational fact-finder to conclude that 'something fishy' was going on, it does not prove beyond a reasonable doubt that defendants reached a concrete agreement to forcibly oppose the United States government."

Defense lawyers said David Stone is a Christian who was preparing to do battle with the armies of the Antichrist, not the U.S. government.

A juror, Rickey Randall, 58, said the judge made the right call.

"I heard talk, talk, talk, but no action," Randall said. "I was shocked by their effort to bring the defendants to trial. ... Do you think a group that small can go up against the mighty U.S. government?"

In her first remarks since the case was gutted, U.S. Attorney Barbara McQuade said she wouldn't have handled the case differently and "strongly" disagrees with the judge's interpretation of federal conspiracy law.

"If you have a suggestion, let me know," she told The Associated Press during an interview in her office.

"I take the view that you stop 'em when you can if you've got evidence," McQuade said. "We have an obligation to protect the public. The jury and the judge don't have that obligation. ... There were many plans to oppose by force the authority of the United States."

David Stone was released on bond after two years in custody. He could face another year in prison when he's sentenced for the gun crime on Aug. 8 but hopes the judge will consider the past two years as enough punishment. Joshua Stone, too, has been jailed for two years and could face more time behind bars.

The elder Stone said the trial's outcome likely will stir anxieties about the government among other militias.

"Now they know their paranoia is true," he said, referring to the FBI's tactics.

David Stone's wife, Tina Stone, and another son, David Stone Jr., were among the militia members cleared of all charges this week. The four Stones walked away from the courthouse together Thursday. Tina Stone earlier said her husband's Hutaree days are over, although she said the group never was the violent threat the government had claimed.

"They couldn't overthrow F Troop," she said, referring to a 1960s TV satire about soldiers in the Old West after the Civil War.

Militia members are finding that resuming their lives isn't easy. Despite his acquittal, Michael Meeks of Manchester, Mich., said he can't open a bank account yet because the bank told him he's still on a terror watch list. Told of the troubles, McQuade didn't seem moved.

"It's the consequences of their actions," the prosecutor told the AP.

Alicia Silverstone Chews Food, Regurgitates It Into Baby's Mouth

Apparently Alicia Silverstone chews food and regurgitates it into her 10-month-old son's mouth to nourish him. We know because she posted a video of this.

Alicia posted the video on her blog this weekend - well played once again, Internet - claiming it's her "favorite" way to feed her son Bear Blu (his real name).

"He literally crawls across the room to attack my mouth if I’m eating," she says, without offering an explanation of why she engages in this behavior. Enjoy:

Is Alicia just trying to spare Bear Blu the arduous task of chewing? Does she believe there is some other health benefit to this? We may never know.

As for what they're eating, Alicia's a staunch vegan and is raising her son to be too, so we're guessing it's some delish soy tofu thing PETA hearts her for.
